This is the reservation form for the Gi Surgical Clinic Kyoto. Gi Surgical Clinic Kyoto is a specialized clinic for same-day inguinal hernia surgery located in Shijo Karasuma, Shimogyo-ku, Kyoto.

Outpatient consultation hours are from 2:00 PM to 6:00 PM on weekdays and from 9:00 AM to 1:00 PM on Saturdays.

We also offer temporary morning consultations on certain days. Please check the calendar for the relevant dates.

Please allow sufficient time for filling out the medical questionnaire and other procedures at the reception.

We have a doctor who can speak everyday English working only on Thursdays.

As our clinic provides insured medical services, those without an insurance card will need to pay for the consultation self-pay. For information regarding the cost, please contact us by phone or email.

For your first visit to our clinic, please bring your health insurance card. If you are currently taking any medications, please bring your medication record book.

● Currently, we only accept cash payments. If you undergo any examinations on the day, the consultation fee plus examination fees might be high, so please come prepared.
